Hopefully you upgraded the boot manager before you did the ipso, otherwise you
corrupted your installation.  The only remedy, and someone correct me if I'm
wrong, would be to delete the initial file for that install under Voyager,
delete the 3.4.1 package and then re-do the install the correct way, upgrading
the boot manager first, and then the ipso, in this manner you get the correct
initial file created.
