Hi back! I recently set up an NG FP2 box and I also had that content disposition problem. The solution is described at http://www.securegateway.org/index.php3?topic=Bugs (I guess, that you can use this hint even for a 4.1 box). I don't know the other problems but try to set the "Match->Host" field to *:* (Look exact! There is a double point between the two asterisks). Maybe this solves your problem.
