I'd second the vote for Mailsweeper. We've used it for years and it is really great.
Tech support is top shelf as well when you need a hand.

For more immediate action, try out some of the websites that test your SMTP server for
open relay such as http://www.abuse.net/relay.html  and see if you pass the tests.
This will give you a better picture perhaps of where you are.
