For whatever resaon the /dev/fw0 device were not created when running cpstart. I ran into the same issue. Try running the script 'fw.mkdev' from the $FWDIR/modules directory. Sometimes I've noticed the same error occuring for VPN-1 module as well. So you might also need to run the 'vpn.mkdev' script.
