[FW-1] URI Specification File works intermittantly

Hello-

I am having a problem with a URI specification file.

I am trying to allow certain users access to certain sites.
My URI file reads:
-
personal.vanguard.com 0
192.175.182.51 0
-
My rulebase is:
userx   >>   Any  >>  Http w/resource allowedsites  >>  accept
-

This seems to work in that I can only access this site from that users pc
and all other sites I get a FW1 Access Denied page, however it will work for
a little while and then it will just stop working inexplicably. I though it
might have something to do with IE caching the page but I cleared that and
still no luck.
Anyone have any thoughts or experience with this?
Using FW4.1 on NT
