As secure as a VPN if as follows:

You open up one port inward (3389 if I remember , well that was for NT 4 TS)
I prefer to use client authentication (via browser)
On the terminal server put the high encryption on (128 bit) and then build
your client disks from there. This will give you a 128bit client
