I had the same problem with Mandrake which is pretty much the same as Red Hat. It
turns out that the pre-filtering mechanism that FW-1 sets up does something funky to
the external network interface and will also produce the error message that you
received when trying to insert the fw module.

An interim solution is to disable the initial filtering mechanism:

#chkconfig --del fw1boot
#reboot

fw1boot is NOT the script that starts the firewall, but is rather a preprocessor
that protects the network interface at boot time.  Removing it does not affect FW1
behavior. When the machine comes back online, you should be able to start FW1
without error (fwstart)
